CASA for Children
This form provides a summary of what a CASA is and does for a child.
CASA for Families
This form provides a summary of what a CASA is and does for biological family members.
CASA for Resource Parents
This form provides a summary of what a CASA is and does for resource parents (foster parents).
CASA for Professionals
This form provides a summary of what a CASA is and does for other professionals (ie. Social Worker, Therapist, Teacher, etc.).
Contact Information Form
You can download and print this form out if you want to have a paper copy to keep all of the contact information for your case.
Visitation Schedule Form
You can download and print this form out as a way to track all of the contact you make with parties of the case.
Basic Questions Form
You can download and print this form to use as a guide when meeting with parties of the case and other professionals involved.
Child Visit Form
This form will help you to record and keep track of what occurs during your visits with the youth on your case. You can download, print, and fill this form out until you can upload or input the information into Optima.
NOTE: You must still input all contact notes into Optima.
Care Fund Request Form
This form is used to make a request for funds when doing activities with the child(ren) on your case. This may be things like going to eat, catching a move, paying for a field trip, etc. Please fill out the form and email it or drop it off to Dave or your Volunteer Coordinator.
Court Report Template
This file can be downloaded and printed to aid you in writing your court report.
Note: Feel free to still schedule a meeting with your volunteer coordinator if you need further assistance.
Court Notes Form
This form can be used to record notes from the court hearing by downloading and printing.
NOTE: You must still submit court hearing information and notes into Optima.
